Profile of the manufacturer including documentary evidence about the company including its Directors/Partners (as the case may be) and authorized signatories.
Documentary evidence such as Certificate of Incorporation from Registrar of Companies or Deed of Partnership, Articles of Association, Memorandum of Association, MSME certificate, etc.,. Other documents, such as GST, PAN registration certificates, shall be submitted.
A service agreement / documentary evidence from the parent manufacturer / original equipment manufacturer (OEM) clearly defining the responsibility, terms, conditions, warranty/guarantee and details of the qualified and trained technical service team assigned with responsibilities of initial installation/commissioning/operation of the apparatus, post-sales services, technical back up, repair & maintenance of the apparatus, supply of original spares, etc. shall be submitted. (if any)
The service agreement shall be signed by authorized representatives of the principal manufacturer. The authorized representatives shall specify the name and designation below the signature. (if any)
The service agreement shall specify the regions covered in India/Pan India basis
A list of reputed customers to whom the apparatus has been supplied nationally or internationally shall be submitted along with a performance report of the apparatus duly endorsed by the end user (only reputed ones).
Quality control setup of the manufacturer at the manufacturing premises. Details of testing facilities available with manufacturer.
Details of equipment and machinery provided for manufacture and stage wise quality assurance including final Quality Control System.
